The Chronic Care Initiative Starting October 2008
Coordinating Agency: The Bureau of Cancer and Chronic Disease in the Department of Health's Community Health Administraton
Funding $10 million that was set aside by City Council for chronic disease management of fatal illnesses and $250,000 from the Preventive Service Block Grant
Aims:

To promote longer and healthier lives at each stage of the illnesses, from detection of early conditions, through living with mild to moderate disease. to living with and dying from advanced illness; and

To deliver highly reliable, evidence based services for persons living with these conditions at the right time and with the lowest possible cost, for all residents of the District of Columbia.
Target Conditions:
Cardiovascular disease, hypertension, diabetes, chronic kidney disease, stroke, and chronic obstructive lung disease.
