The renewal process consists of the following steps:
1. License Renewal Application
2. Payment by credit card (Mastercard or Visa) or check (paper renewal only)
You may complete the renewal process online.
You may request a paper form be mailed to you by calling 1-877-672-2174. If you decide to complete a paper renewal, please print all answers and mail the completed renewal to the address below:

PAID INACTIVE REQUESTS
To request your license be placed on Paid Inactive status, a paper renewal must be completed. Contact customer service at 1-877-672-2174 for more information.
ATTENTION ALL APPLICANTS FOR A LICENSE, CERTIFICATE, OR REGISTRATION: “The term for a certificate, license, or registration issued or renewed…shall be two (2) years or for the balance of the license period, whichever is shorter.” Title 17 Chapter 40, Section 4006.1
